Summary of Project Objectives

Consultants spend 4 weeks (no more than 40 hours per week, max 160 hours a month) onsite at a skilled or non-skilled long-term care facility.  Up to 8 hours per week can be dedicated to remote work.  

The IP will: 

  • Complete a facility assessment throughout the four weeks.  

  • Submit weekly summary reports covering findings and progress made on improvements. A sample of a completed report can be found here.

  • Conduct educational sessions for staff on all shifts. Document educational offerings on the APIC Consulting Sign-in Sheet or utilize the facility’s sign-in sheet

    • Capture the subject of the education session and staff disciplines of attendees. 

  • Conduct audits/observations for staff on all shifts.  Focus on priority audits such as Hand Hygiene, PPE, and Cleaning of Shared Equipment. 

  • Revise or create policies based on facility needs.  Document these on Policy Revision Sheet.
