Assessment and Tracking Tool – NJ DOH ICAR

NJ DOH requested that APIC Consulting develop a tool that is not an ICAR to assess the facilities in this project. The NJ DOH LTC Assessment and Tracking Tool (ATT) is ready for your next facility assignments!   

  • The ATT will be completed IN ADDITION to the weekly findings summary report.  

  • This assessment covers many of the same domains as the CDC ICAR but has sections for free text observations.  

  • Consultants will complete the ATT once throughout the 4-week assignment.   

    • Consultants will receive the electronic link to the ATT 

    • A pdf will also be provided if consultants initially prefer to complete the assessment on paper 

  • The survey link has a Table of Contents that allows the consultant to navigate to various topics 

  • The ATT does not have to be completed all at once.   

    • The final section of the Table of Contents is “Retake Link.” Navigate to that section and select “no” to the question “Are you finished?” 

    • The next page will prompt you to enter your email address.  A NEW link will be emailed to you.  This link will take you to the ATT that you started with all of your notes saved.  

    • Once completed, select “yes” to the question “Are you finished?”. Please be aware that you can no longer access the survey once you answer “yes” and hit submit. 

  • Consultants are encouraged to review the free-text questions and provide updates as their assignment progresses.   

  • The first required field is for the Facility ID number 

    • This will be provided to you before the facility kick-off call and used in place of the facility name.
